We would like to know how we can improve our service to you and how you perceive our surgery and staff.
To help us with this, we are setting up a virtual patient reference group so that you can have your say. We will ask the members of this representative group some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We will contact you via email and keep our surveys succinct so it shouldn’t take too much of your time.
We aim to gather around a hundred patients from as broad a spectrum as possible to get a truly representative sample. We need young people, workers, retirees, people with long term conditions and people from non-British ethnic groups.
If you are happy for us to contact you occasionally by email please click the link below to open the sign-up form and complete all the fields.

Meeting – Tuesday 10th September 2019, 6pm Shiregreen Medical Centre.
We are actively seeking more Patients from both our Shiregreen and Melrose surgeries to become involved in our new Patient Participation group. We have had some interest and we will be meeting with the newly formed PPG on 10th September.
This is a newly formed group and you will work alongside some of the Doctors and Staff and you will help represent our Patients in discussions about improving our services.
We are looking for patients from all parts of our communities to share their views and opinions and help us shape the future of your care and surgeries.
If you are interested and want more information please ask to speak to the Practice Manager, Rachel or you can contact her directly by email email@example.com
A recent survey was run by the Patient Participation Group. A copy of the report is available below.
