Patient Survey 2017 coming soon

New Patient Registration

If you wish to register as a new patient with the practice, please check at reception that you live within the practice boundary area. New patient registrations are only accepted subject to seeing 2 forms of Identity to verify your name and your address. This must be proof of current address and photo identification, please ask reception for more details.

All new patients are asked to fill out a registration form and must attend a new patient health check appointment with the healthcare assistant. You will not be registered until you have attended, but may be seen by a doctor as a temporary resident if the doctor considers it to be urgent and necessary.

If you move house to outside of the practice boundary area, we are required to ask you to leave our surgery to register with another surgery immediately.
